08bd4fc156 mm/hugetlb: remove arch_prepare/release_hugepage from arch headers
Nobody used these hooks so they were removed from common code, and can now
be removed from the architectures.

2ae416b142 mm: new mm hook framework
CRIU is recreating the process memory layout by remapping the checkpointee
memory area on top of the current process (criu).  This includes remapping
the vDSO to the place it has at checkpoint time.

However some architectures like powerpc are keeping a reference to the
vDSO base address to build the signal return stack frame by calling the
vDSO sigreturn service.  So once the vDSO has been moved, this reference
is no more valid and the signal frame built later are not usable.

This patch serie is introducing a new mm hook framework, and a new
arch_remap hook which is called when mremap is done and the mm lock still
hold.  The next patch is adding the vDSO remap and unmap tracking to the
powerpc architecture.

This patch (of 3):

This patch introduces a new set of header file to manage mm hooks:
- per architecture empty header file (arch/x/include/asm/mm-arch-hooks.h)
- a generic header (include/linux/mm-arch-hooks.h)

The architecture which need to overwrite a hook as to redefine it in its
header file, while architecture which doesn't need have nothing to do.

The default hooks are defined in the generic header and are used in the
case the architecture is not defining it.

In a next step, mm hooks defined in include/asm-generic/mm_hooks.h should
be moved here.

d70b3ef54c Merge branch 'x86-core-for-linus' of git://git.kernel.org/pub/scm/linux/kernel/git/tip/tip
Pull x86 core updates from Ingo Molnar:
 "There were so many changes in the x86/asm, x86/apic and x86/mm topics
  in this cycle that the topical separation of -tip broke down somewhat -
  so the result is a more traditional architecture pull request,
  collected into the 'x86/core' topic.

  The topics were still maintained separately as far as possible, so
  bisectability and conceptual separation should still be pretty good -
  but there were a handful of merge points to avoid excessive
  dependencies (and conflicts) that would have been poorly tested in the
  end.

  The next cycle will hopefully be much more quiet (or at least will
  have fewer dependencies).

  The main changes in this cycle were:

   * x86/apic changes, with related IRQ core changes: (Jiang Liu, Thomas

     - This is the second and most intrusive part of changes to the x86

       This now reflects the actual hardware and allowed us to distangle
       the domain specific code from the underlying parent domain, which
       can be optional in the case of interrupt remapping.  It's a clear
       separation of functionality and removes quite some duct tape
       constructs which plugged the remap code between ioapic/msi/hpet
       and the vector management.

     - Intel IOMMU IRQ remapping enhancements, to allow direct interrupt

       This is an important RAS feature which adds hardware support for
       poisoned data.  That means roughly that the hardware marks data
       which it has detected as corrupted but wasn't able to correct, as
       poisoned data and raises an APIC interrupt to signal that in the
       form of a deferred error.  It is the OS's responsibility then to
       take proper recovery action and thus prolonge system lifetime as
       far as possible.

     - Add support for Intel "Local MCE"s: upcoming CPUs will support
       CPU-local MCE interrupts, as opposed to the traditional system-
       wide broadcasted MCE interrupts (Ashok Raj)

     - Misc cleanups (Borislav Petkov)

32e55a777f ARM: 8389/1: Add cpu_resume_arm() for firmwares that resume in ARM state
Some platforms always enter the kernel in the ARM state even if
the kernel is compiled for THUMB2. Add a small wrapper on top of
cpu_resume() that switches into THUMB2 state.

This provides the functionality to fix a problem reported by Kevin
Hilman on next-20150601 where the ifc6410 fails to boot a THUMB2
kernel because the platform's firmware always enters the kernel in
ARM mode from deep idle states.

8a14849b4a arm64: KVM: Switch vgic save/restore to alternative_insn
So far, we configured the world-switch by having a small array
of pointers to the save and restore functions, depending on the
GIC used on the platform.

Loading these values each time is a bit silly (they never change),
and it makes sense to rely on the instruction patching instead.

This leads to a nice cleanup of the code.

556269c138 arch/*/io.h: Add ioremap_wt() to all architectures
Add ioremap_wt() to all arch-specific asm/io.h headers which
define ioremap_wc() locally. These headers do not include
<asm-generic/iomap.h>. Some of them include <asm-generic/io.h>,
but ioremap_wt() is defined for consistency since they define
all ioremap_xxx locally.

In all architectures without Write-Through support, ioremap_wt()
is defined indentical to ioremap_nocache().

frv and m68k already have ioremap_writethrough(). On those we
add ioremap_wt() indetical to ioremap_writethrough() and defines
ARCH_HAS_IOREMAP_WT in both architectures.

The ioremap_wt() interface is exported to drivers.

b01aec9b2c EDAC: Cleanup atomic_scrub mess
So first of all, this atomic_scrub() function's naming is bad. It looks
like an atomic_t helper. Change it to edac_atomic_scrub().

The bigger problem is that this function is arch-specific and every new
arch which doesn't necessarily need that functionality still needs to
define it, otherwise EDAC doesn't compile.

So instead of doing that and including arch-specific headers, have each
arch define an EDAC_ATOMIC_SCRUB symbol which can be used in edac_mc.c
for ifdeffery. Much cleaner.

And we already are doing this with another symbol - EDAC_SUPPORT. This
is also much cleaner than having CONFIG_EDAC enumerate all the arches
which need/have EDAC support and drivers.

This way I can kill the useless edac.h header in tile too.

Some platforms might not be able to fully utilize VFP when e.g: one CPU
out of two in a SMP complex lacks a VFP unit. Adding code to migrate

The !CONFIG_SMP implementation of futex_atomic_op_inuser() seems to rely
on disabled preemption to guarantee mutual exclusion.

From commit e589ed23dd ("[ARM] 5218/1: arm: improved futex support"):

    "For UP it's enough to disable preemption to ensure mutual exclusion..."

From the code itself:

    "!SMP, we can work around lack of atomic ops by disabling preemption"

Let's make this explicit, to prepare for pagefault_disable() not
touching preemption anymore.

If the 8250 debug code is used in word mode on an big endian
host then the writes need to be change into little endian for
the bus.

Note, we have to re-convert the value back as the debug code
will inspect the value after writing it to see if a newline
has been written.

8746515d7f xen: Add __GFP_DMA flag when xen_swiotlb_init gets free pages on ARM
Make sure that xen_swiotlb_init allocates buffers that are DMA capable
when at least one memblock is available below 4G. Otherwise we assume
that all devices on the SoC can cope with >4G addresses. We do this on
ARM and ARM64, where dom0 is mapped 1:1, so pfn == mfn in this case.

No functional changes on x86.

1424532b21 ARM: 8347/1: dma-mapping: fix off-by-one check in arm_setup_iommu_dma_ops
Patch 22b3c181c6 ("arm: dma-mapping: limit
IOMMU mapping size") added a check for IO address space size. However
this patch broke IOMMU initialization for typical platforms initialized
from device tree, which get the default IO address space size of 4GiB.
This value doesn't fit into size_t and fails a check introduced by that
commit resulting in failed dma-mapping/iommu initialization. This patch
fixes this issue by adding proper support for full 4GiB address space
size.

fd1d0ddf2a KVM: arm/arm64: check IRQ number on userland injection
When userland injects a SPI via the KVM_IRQ_LINE ioctl we currently
only check it against a fixed limit, which historically is set
to 127. With the new dynamic IRQ allocation the effective limit may
actually be smaller (64).
So when now a malicious or buggy userland injects a SPI in that
range, we spill over on our VGIC bitmaps and bytemaps memory.
I could trigger a host kernel NULL pointer dereference with current
mainline by injecting some bogus IRQ number from a hacked kvmtool:
